Ways to Make a Legacy Gift
There are many ways to include CBI in your long-term plans:
Bequest in your will or trust
I/we bequeath to Combat Blindness International (tax ID# 39-1531546), located in Madison, Wisconsin, _______% of my residual estate – OR – the sum of $_______ to be used for charitable purposes in furthering the mission of Combat Blindness International.
Beneficiary designations
Name Combat Blindness International as a beneficiary of:
A retirement account
A life insurance policy
A bank or investment account (payable-on-death or transfer-on-death designation)
Charitable gift annuity
Receive income during your lifetime while supporting CBI’s mission in the future.
